What Do I Need to Enroll?

Whether you're enrolling online, over the phone, or in-person there is certain information you'll need to have available.

To Enroll in Medicaid or CHIP You'll Need:

  • Names, dates of birth, and Social Security Numbers for all those applying
  • Pay stubs from the last 3 months for anyone working in your household
  • Information about health insurance offered through your job

To Enroll in the Health Insurance Marketplace You'll Need:

  • Names, dates of birth, and Social Security Numbers for all those applying
  • Estimated annual household income (to see if you qualify for a discount)
  • Information about health insurance offered through your job

The Health Insurance Marketplace has many different options to choose from. To make the best decision for you and your family, consider these questions when selecting a plan:

  • Do I have a doctor that I want to continue seeing? You will need to make sure that your insurance plan's provider network includes the doctors or hospitals that you want to see.
  • Am I expecting a lot of medical expenses this year? If so, you may consider selecting a plan with a lower out-of-pocket maximum even if it means that you will have a higher premium (monthly cost).
  • Am I taking prescription drugs regularly? All plans in the marketplace have coverage for prescription drugs, but each plan may differ in how they share the cost. Check the plan's brochure to ensure that you get a great value for your prescription drug needs.
